What is Lexapro?

Lexapro, which contains the active ingredient escitalopram, is a selective serotonin reuptake inhibitor (SSRI) that is used to treat major depressive disorder (MDD), generalized anxiety disorder (GAD), and https://www.nimh.nih.gov/health/topics/obsessive-compulsive-disorder-ocd/index.shtmlobsessive compulsive disorder (OCD). Escitalopram is a citalopram’s S-enantiomer and is approximately 150 times more potent than citalopram’s R-enantiomer. Compared to other drugs in this class, escitalopram is more selective for the serotonin transporter which may be why this medication causes fewer side effects.

Lexapro: Mechanism of Action

Lexapro, escitalopram, increases serotonin levels in the brain by blocking the reabsorption of serotonin into the presynaptic neuron. In doing so, serotonin stays longer in the synaptic gap and stimulate the recipient cell. How do I take Lexapro?

Lexapro is usually taken 10 mg - 20 mg once daily, at the same time each day, although the dose may be adjusted depending on your medical condition or your health. It may take several weeks for you to feel the effect of using Lexapro and the improvement can be gradual. It is important to continue to take Lexapro at the dose your doctor recommends and for as long as your doctor recommends. You should continue to take this medication regularly even if you start to feel better unless told by our doctor otherwise.

If you forget to take a dose, take your next dose according to your usual schedule and do not double dose in the same day.

Lexapro and withdrawal

If you're tolerating an antidepressant such as Lexapro well and it's helping you enjoy life with renewed energy, your doctor may want you to continue your treatment indefinitely to prevent relapse. However, some patients still would like to discontinue their medication because of unwanted side effects or they may feel they no longer need the medication.

Antidepressant discontinuation syndrome is a common condition that happens to 10% of patients who decide to stop their therapy. Symptoms of antidepressant withdrawal can be similar to symptoms of anxiety and depression. You may experience nausea/vomiting, cramps, loss of appetite, flushing, dizziness, trouble sleeping, irritability, and numbness. Many of these symptoms can be minimized by tapering over weeks or months. They can start days-weeks of changing your dose. SSRIs such as Lexapro are associated with less withdrawal symptoms. It’s important to note that antidepressant discontinuation syndrome is not the same as being addicted to an antidepressant. Addiction is when a patient seeks increasingly higher doses but that normally does not occur with antidepressant patients. Discontinuation syndrome, or “withdrawal”, is due to your brain adjusting to different levels of neurotransmitters.

What are the side effects of taking Lexapro?

Some common Lexapro side effects are: nausea, fatigue, fever, constipation/diarrhea, sweating, changes in sleep or appetite, weight gain, tremors, drowsiness, dry mouth, heartburn, and decreased sexual libido. Most of these side effects are mild and passing. Most patients do not need additional therapy to manage their side effects. You may experience these side effects if you forget your dose or if you reduce your dose so please talk to a doctor before making changes to your dose regimen. If side effects do not improve or disappear after a few days, please talk to your doctor.

Can I take Lexapro with an empty stomach?

Is it safe to drink alcohol while taking Lexapro?

The general recommendation is to avoid alcohol while taking any antidepressant because alcohol can make anxiety and depression worse and more challenging to treat. Drinking alcohol can also worsen Lexapro’s side effects such as dizziness and drowsiness.

What drugs interact with Lexapro?

There are many interactions with Lexapro and the following is not a complete list. Please speak with your health care provider and individually assess all the medication that you are using.

Medications that interact with Lexapro include: MAOIs, pimozide, linezolid, methylene blue, antiarrhythmics, antipsychotics, opioids, diuretics, laxatives, other SSRIs, lithium, tryptophan, cimetidine, triptans for migraine, fluconazole, ketoconazole, itraconazole, omeprazole, St John’s wort, anticoagulants, pain medications, and some cough and cold preparations. Please note, this list is not complete.

Is Lexapro and Cipralex the same thing?
Escitalopram, the active ingredient in Lexapro, is available in many countries using different names. In USA, brand name Lexapro is manufactured by Allergan. But did you know that the original active ingredient was discovered by a company named Lundbeck? While Lundbeck co-markets with Allergan for Lexapro in USA, they are the solo-marketer for Cipralex in Canada. Both Cipralex and Lexapro use the same, original, formulation discovered by Lundbeck. Other than their names, they are the same products.
Lexapro weight loss or weight gain
Weight gain is a common side effect of many antidepressants because they seem to alter serotonin-based regulation of appetite function. This could increase your cravings for carbohydrate snacks or make you feel the need to eat even when you’re full. Some studies show that SSRI use may have a short-term weight loss effect, but over long term, it causes weight-gain. Fortunately, if you gain weight, many people weight loss occurs after discontinuation of the drug as your body’s serotonin levels stabilize and the food cravings decrease. However there are no long-term studies that follow patients after they stop therapy to assess their weight gain/loss.
Can you overdose on Lexapro?
Lexapro overdose can occur whether it’s taken alone or in combination with other medication. When used alone, Lexapro overdose is rarely fatal. Lexapro is usually taken 10mg or 20mg daily. Cases where overdoses of 140mg daily have been reported where the patients were hospitalized and had abnormal heart rate, altered mental state, and other symptoms. However, of the 79 cases documented in 2009, all the patients survived.
Lexapro vs Prozac, Wellbutrin, Celexa, Effexor
Prozac (fluoxetine) is another medication that is used to treat anxiety and depression. Compared to Lexapro, Prozac may cause less fatigue and it doesn’t have as many cardiac side effects. It causes relatively little weight gain but it tends to take longer to feel an effect compared to other options. Wellbutrin (bupropion) causes less sexual side effects than other antidepressants and may even boost sex drive in some patients. Unlike other antidepressants, Wellbutrin can have a some energizing and stimulating effects which may lead to anxiety or insomnia for some people. It can be used to help with smoking cessation. Wellbutrin is less likely to cause weight gain but has a higher risk of causing seizure. This is one of the reason why it’s more important to not drink alcohol with Wellbutrin versus other antidepressants. Celexa (citalopram) does not cause as much drowsiness as other options and has a minor effect on weight gain. Celexa can also be used to help with alcoholism, hot flashes, and binge-eating disorder. Effexor (venlafaxine) can cause more antidepressant withdrawal symptoms and drowsiness in relations to other antidepressants. Unlike Lexapro, venlafaxine affects two neurotransmitters so it is a viable option if a patient fails to improve on Lexapro.
Is Lexapro safe during Pregnancy?
Some women chose to discontinue their antidepressants during pregnancy while for others, the benefits of staying on an antidepressant outweigh the risks. Always speak with your doctor about your concerns before discontinuing or changing your Lexapro. Some evidence suggest that untreated depression during pregnancy can increase the risk of preterm delivery, low birthweight, preeclampsia, miscarriage, and/or postpartum depression.
