Wellbutrin XL, which contains the active ingredient bupropion, is classified as an antidepressant. Wellbutrin XL is used to treat major depressive disorder (MDD) and prevention of seasonal major depressive episodes in patients 18 years and older. Use of Wellbutrin XL should be avoided in patients with a history of seizure disorders or patients with bulimia or anorexia nervosa due to its potential to increase the risk of seizures. Wellbutrin XL should also not be used in patients undergoing withdrawal from sedatives, alcohol, or benzodiazepines.
How does Wellbutrin XL work?
The exact mechanism of Wellbutrin XL is not clearly understood. Leading experts believe that Wellbutrin XL works by inhibiting the reuptake of dopamine and norepinephrine from synapses between neurons. This helps to resolve the imbalance of neurotransmitters that are released from patients with depression. The mechanism of action of Wellbutrin XL is unique amongst antidepressants because of its effects on dopamine – other antidepressants such as SSRIs (selective serotonin receptor inhibitors) and TCAs (tricyclic antidepressants) are not documented to have clinical significant effects on dopamine. How do I use Wellbutrin XL?
Patients new to Wellbutrin XL usually begin therapy at 150mg daily in the morning. After 1 week of treatment, your doctor may want to increase your dose to 300mg daily in the morning or maintain 150mg daily if you experience side effects with the higher dose. Patients who use Wellbutrin XL seasonally usually start their treatment in fall, before any onset of system, continue their medication through winter, and gradually taper and discontinue doses by spring.
If you forget a dose, skip the dose and take your next dose the following day at your regularly scheduled time. Do not take more than 1 dose per day.
What's the difference between Wellbutrin XL and Wellbutrin SR?
Wellbutrin XL contains bupropion extended release whereas Wellbutrin SR contains bupropion sustained release. Despite their similar names, these medication are not interchangeable and each have their own therapeutic advantages. Wellbutrion XL is dosed once daily and does not have a generic available. Because Wellbutrin XL stays in your body over a longer period with fewer fluctuations in concentrations, it causes fewer side effects. Wellbutrin SR is taken twice daily and there are generics available. Studies have shown that once daily dosing of Wellbutrin XL is bioequivalent to twice daily dosing of Wellbutrin SR and three-times daily dosing of bupropion regular release tablets.
What are common side effects of Wellbutrin XL?
Common side effects of Wellbutrin XL are dry mouth, constipation, nausea, insomnia, anxiety, dizziness, and decreased appetite. Some patients may also experience flatulence, headache, or sinusitis. Some rare, but serious, side effects you should watch out for include: palpitations, chest pain, rash, blurred vision, anaemia, irregular heart rate, irregular menstruation, genital rash, erectile dysfunction, wheezing, swelling, and peripheral coldness. Can I drink alcohol when taking Wellbutrin XL?
As is the case with most antidepressants, it is not advisable to use Wellbutrin XL and drink alcohol at the same time. Although alcohol does not have a direct interaction with Wellbutrin XL, it does increase the risk of side effects, including increasing your risk of seizures, hallucinations, suicidal thoughts, and anxiety. At the same time, sudden discontinuation of alcohol, if you are a regular drinker, is not recommended either as it can similarly increase your risk of seizures.

Wellbutrin XL is metabolized by CYP2B6 and has interactions with all other drugs metabolized by this enzyme such as, but not limited to, most antidepressants, beta-blockers, antipsychotics, and antiarrhythmics.
