About Zytiga 500 mg

What is Zytiga?

Zytiga, which contains the active ingredient abiraterone, is a androgen biosynthesis inhibitor. Zytiga is used with prednisone for treatment of mCRPC (metastatic castration resistant prostate cancer) if patients have failed androgen deprivation therapy and have tried docetaxel.

Zytiga can also be used with prednisone AND androgen deprivation therapy if the patient is at high risk of metastatic prostate cancer.

Zytiga is used to slow tumor growth or delay need for additional drugs to help manage cancer pain.

How does Zytiga work?

At its core, Zytiga is an androgen biosynthesis inhibitor which means it stops your body from making testosterone by blocking an enzyme called CYP17. Decreasing your body’s level of testosterone causes the growth of cancer to slow down or to shrink. By blocking CYP17, Zytiga not only decreases your body’s production of androgen from the testes but also from the adrenals and the tumor.

How should I take Zytiga?

Zytiga is usually dosed at 1000mg once a day. It is crucial to take Zytiga on an empty stomach. As food can cause Zytiga levels to be an 10 to 17 times higher.

If you miss one dose of Zytiga (or prednisone), take your usual dose the next day. If you miss 2 doses or more, speak with your doctor immediately.

What's the difference between Zytiga and Xtandi?

Zytiga vs Xtandi; Zytiga and Xtandi have not be compared in head to head trials so it's not a clear choice which therapy to try first. Current information shows that both drugs appear to provide similar disease management so the choice of which agent to use is based on patient factors and side effects. Both Zytiga and Xtandi will cause fatigue and cognitive impairment.

Zytiga plus prednisone is not preferred for patients with diabetes, heart failure, or fluid overload as it can lead to hypokalemia and will require potassium and liver function monitoring. Xtandi is not preferred for patients with seizures or are taking seizure-lowering medications. Outside of these patient extremes, the choice of Zytiga or Xtandi may be down to which therapy your doctor is more comfortable with and has more experience with.

There isn't a general consensus on which medication is better tolerated and there isn't evidence that it's better to start on one over the other.

Common side effects of taking Zytiga

Not everyone experiences the same severity or the same side effects. There are some very common side effects for Zytiga which includes constipation, diarrhea, fatigue, cough, hot flashes, vomiting, insomnia, anemia and high blood pressure.

Less commonly, patients can experience headache, depression, bruising, falls, increased urinary frequency, presence of blood in urine, fractures/falls, weight gain, respiratory tract infections, irregular heart rate, heart failure, high levels of lipids in blood, or abnormal liver function tests.

Rarely, Zytiga can also cause adrenal gland problems.

Can I take Zytiga with an empty stomach?

It is important to take Zytiga on an empty stomach (i.e. do not eat for at least 2 hours before the dose or at least 1 hour after the dose).

Can I drink alcohol while taking Zytiga?

There isn’t a direct interaction between alcohol and Zytiga but it is still advisable to avoid alcohol during therapy. There are 2 reasons for this recommendation. Firstly, Zytiga has the potential to cause liver toxicity and that risk may increase if patients drink too much alcohol during therapy. Secondly, and more commonly, Zytiga causes hot flashes and this side effect can be triggered by alcohol consumption.

What drugs should I avoid when taking Zytiga? Zytiga drug interactions

Zytiga inhibits CYP2C8 and CUP2D6 and is metabolized by CYP3A4 and SULT2A1. This means that Zytiga has numerous drug interactions with other drugs that are metabolized by the CYP2C8 and CUP2D6 pathways. Please speak with your doctor/healthcare provider and provide them with all the medication you are taking to discuss each medication individually.

There are over the counter medication to avoid as well (such as St John’s wort and common cough and flu preparations) so please include that in your medication history when speaking with your doctor.
