About Pradaxa 75mg

What is Pradaxa used for?

The active ingredient in Pradaxa is Dabigatran Etexilate. Pradaxa belongs in the drug class called anticoagulant - commonly known as blood thinners. Pradaxa indications include prevention of VTE (venous thromboembolic events) in patients who had hip replacement or knee replacement surgery, treatment and prevention of DVT (deep vein thrombosis) and pulmonary embolism, and the prevention of strokes/embolisms in patients with afib (atrial fibrillation).

What is the mechanism of action of Pradaxa?

Pradaxa (Dabigatran) is a reversible direct thrombin inhibitor which prevents the conversion of fibrinogen to fibrin and thus prevents the development of thrombus. The half-life of Pradaxa is about 12-14 hours and is independent of dose.

The bioavailability of Pradaxa is greatly increased when the pellets are taken without the outer capsule shell - therefore, to avoid this, Pradaxa should always be taken whole. Do not open the capsules and take the pellets alone or sprinkled over food/into beverages.

What is the dosage of Pradaxa?

Speak to your doctor about your kidney functions before starting Pradaxa as dosage adjustments will be required if you have renal impairment. In general, if you're using Pradaxa post-surgery, therapy should start 1-4 hours after the surgery is completed. The maintenance dose for knee or hip replacement surgery is 220mg daily for 10 to 35 days. For treatment or prevention of DVT and PE, doses can vary from 110mg twice daily to 150mg twice daily.

If you forget and miss your dose, do not double dose to make up for the missed dose. In most cases, you can take a dose of Pradaxa up to 6 hours prior to your next scheduled dose or continue with regularly scheduled doses. Depending on what you're using Pradaxa for, the pharmacist will have different advice.

Pradaxa Alternatives and their Scope of Use

Alternatives to Pradaxa include Plavix, Eliquis, Xarelto, and Coumadin.

To clear the air, "Plavix" is an "anti-platelet agent" while the other drugs in the list are "blood thinners". This technicality affects prescriber choice far more than patient choice. For patients who do not want to get into the technicalities, just know that they're not the same thing and they're not interchangeable. The differences between Pradaxa, Eliquis, and Xarelto are much less distinct. These agents are classified as new direct anticoagulants.

Studies have shown that new direct anticoagulants work just as well as Coumadin (warfarin) but comparing these agents head to head is a much more difficult task. Other than considering the specific indication, and whether current medical studies show any safety or efficacy benefit between the three choices, a lot of the patient/prescriber choice boils down to individual patient lifestyles and health conditions. For example, for non-compliant patients, prescribers may consider Xarelto which is dosed once daily versus the twice daily requirement of Eliquis and Pradaxa.

Pradaxa is known for causing more stomach upset and is more affected by poor renal function so if a patient gets nauseous easily, it may not be the best choice. Eliquis can cause more gastric bleeding than the other drugs so avoid in patients who are already at increased risk. Overall, these new direct anticoagulants are increasingly popular because of their low need for regular lab monitoring compared to the old-school-gold-standard, warfarin. Although warfarin requires regular (usually once/month) lab tests, it still has a place in modern anticoagulant therapy for patients who have severe liver or kidney dysfunction.

What are the side effects of Pradaxa?

The majority of Pradaxa side effects are related to signs of bruising or bleeding. On rare cases, it can cause serious or severe bleeding and if that occurs at any part of your body, seek immediate medical attention. For this reason, if you fall, hit your head, or injure yourself in any other way, please seek immediate medical attention.

Can I take Pradaxa on an empty stomach?

Pradaxa can be taken on an empty stomach or with food.

Is it safe to drink alcohol when taking Pradaxa?

There is no direct interaction between Pradaxa and alcohol but drinking regularly may increase your risk of stomach bleeding. Please talk to your doctor to find out how much alcohol is safe for you.

Pradaxa nursing implications

There are no data available to support the use of Pradaxa while nursing. As a result, please talk to your doctor about either discontinuing nursing or discontinuing the drug.
